As I understand AH does introduce random failures based on the a/c current condition.  I was wondering what type of failues are included, (such as flaps, brakes, nav, engine etc)

The failures are mainly instruments and a few systems.  They always put the pucker meter deep in the yellow.

You can cause flap failure by extending flaps above the listed blow out/damage speed that is listed in the AC config.files.
