Transitions?

   Hi guys.

I know there is a transition altitude which in the US is 18000 feet
but differs in Europe(dont know if its a set alt like 5000 or 6000 feet though?)
its when an aircraft should change its qnh from above sea level to standard above FL18 preassure.

But then there is transition in sids and stars as well, what are these transitions? i hope someone can shed some light on this?

For instance on the Turbo 5 dep from KSTL there are 6 transitions. What do you transform from and to what and why?
